What Shimatsu Forms is

Shimatsu Forms is a service for filling in applications, recruitment forms, requests and other forms. Filling in a form is free and needs no account. The service is operated by:

Operator
Tomáš Zatloukal
Address
Jana Maluchy 42/8
Data protection officer
privacy@shimatsu.eu

Filling in forms

  • Answer truthfully and for yourself. Give other people’s personal data only when you have a reason to.
  • A form may be closed, limited in time or limited in the number of responses.
  • Sending a form does not entitle you to acceptance or a positive outcome. The team running the form decides.

Tracking code and private link

After sending you get a code that shows the status of your response. If the form also issues a private link, it opens your whole response and, depending on the form, lets you message the team or edit the response. Anyone with the link can do the same as you, so do not share it.

What is not allowed

  • sending illegal, threatening or abusive content,
  • pretending to be someone else,
  • uploading harmful files,
  • sending forms in bulk or automatically, or getting around spam protection,
  • trying to reach other people’s responses or disrupt the service.

We may delete such responses and limit further sending. If you find a security issue, write to support@shimatsu.eu.

Your content

What you send stays yours. You allow the operator to store it and use it to handle your response. How we treat personal data is described on the Privacy notice page.

Operation and liability

We provide the service as it is and do not guarantee that it is always available. We are liable for damage to the extent set by law. Statutory consumer rights are not affected.

Changes

We may change these terms. We publish the new version on this page with a new effective date.

Law and contact

These terms are governed by the law of the Czech Republic. Send questions to support@shimatsu.eu.
